Nurse Practitioner ( NP ) for our Skilled Nursing Facility ( SNF )

Compensation: $102,000 - $138,000 Annual Salary

Position Description:

 

We are looking for a dedicated and compassionate Nurse Practitioner ( NP ) for our Skilled Nursing Facility ( SNF ) in Newark, OH. The NP will serve as a primary medical provider for assigned residents, working closely with the Medical Director, nursing staff, and the interdisciplinary care team. This is a full-time, in-person position, Monday through Friday daytime hours. No weekends, no nights, no on-call.

Qualifications of the Nurse Practitioner ( NP ):

 

Active Ohio NP license in good standing

National board certification (FNP or AGNP)

SNF, geriatric, or post-acute experience preferred

Strong clinical judgment and ability to work independently

 

Responsibilities of the Nurse Practitioner ( NP ):

 

Conduct comprehensive assessments, rounding visits, and follow-up care for residents

Diagnose and manage acute and chronic conditions common in the SNF population

Collaborate with the Medical Director, nursing staff, and care team on treatment plans

Document thoroughly and timely in the EMR
