About the Role:

ComForCare Home Health Care (North San Diego) is looking for a compassionate and dedicated LVN Field Nurse to join our growing team in San Diego, CA. Responsibilities:

Provide skilled nursing care to home health patients in accordance with physician-approved care plans

Conduct patient assessments, monitor vital signs, and document clinical findings accurately

Administer medications, wound care, IV therapy, and other treatments as ordered

Educate patients and family members on disease management, medications, and home safety

Collaborate with physicians, RNs, and interdisciplinary care team members to ensure continuity of care

Maintain accurate and timely clinical documentation in compliance with state and agency standards

Travel to patient homes throughout the North San Diego service area

Requirements:

Active and unrestricted LVN license in the state of California

Minimum 1 year of clinical nursing experience; home health experience preferred

Current CPR/BLS certification

Valid California driver's license, reliable transportation, and active auto insurance

Strong clinical assessment, wound care, and medication administration skills

Excellent communication, time management, and organizational abilities

Compassionate, self-motivated, and comfortable working independently in the field
