We foster a supportive culture where every team member is empowered to grow, contribute, and make a meaningful impact on the lives of patients, families, and the communities we serve.What We Offer:A $5000 sign-on bonus: Paid quarterly in payments of $1250Mileage ReimbursementCompetitive salaryComprehensive benefits package, including:EAP401(K) with 4% matchOngoing professional development and career advancement opportunitiesA collaborative, mission-driven team committed to delivering exceptional patient careLocation: Riverside Central and EastAs a Home Health OT, you will:Provide skilled occupational therapy services in accordance with physician orders and the patient's individualized Plan of Care (POC).Evaluate patients' functional abilities and assist the physician and care team in determining appropriate treatment goals and interventions.Contribute to the development and ongoing updates of the patient's Plan of Care based on functional needs and progress.Assess the home environment and recommend appropriate modifications, equipment, or strategies to promote safety, independence, and quality of life.Monitor patient progress and communicate relevant changes and recommendations with the physician, case manager, patient, and family/caregivers.Educate and collaborate with patients, families, and LORIAN team members regarding therapy programs, techniques, and strategies to support continued progress.Maintain accurate, timely clinical documentation in accordance with LORIAN policies and procedures, while participating in continuing education and in-service programs.Required QualificationsGraduate of an occupational therapy school accredited by the Council of Medical Education of the AMA in collaboration with the American Association of Occupational Therapists and/or by the State Department of Education in the specific state where employed.Registered by the Occupational Therapist Association and licensed in the state of practice if the state has registered practices.Valid driver's license and insured personal vehicle (per Lorian requirements).Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.Possesses and maintains current CPR certification.This position is based on a pay per visit compensation model.
