Albany Physical Therapist

The Physical Therapist (Outpatient/Inpatient) works under the supervision of leadership. Evaluates, plans and implements treatment programs for the rehabilitation of patients, as directed by a provider, using approved treatment procedures. Provides patient/family education. Participates in family conferences. Responsible for all necessary documentation as required by accepted medical guidelines. Responsibilities Evaluates patients referred by providers. Establishes goals that maximize the patient’s recovery. Plans a treatment program, using appropriate treatment procedures that will best accomplish the established goals. Implements the treatment program using approved procedures. Documents treatment procedures and the patient’s response to them on a regular basis. Responsible for treatment procedures, documentation, and supervision of physical therapy assistants, physical therapy students, and rehab aides, as appropriate. Participates in staff meetings, family conferences and interdisciplinary teams meetings, as appropriate. Participates in continual process improvement. Other duties as assigned Education Bachelor's Degree - Graduate of accredited school of Physical Therapy - Required Licenses and Certifications Physical Therapist (PT) - Licensed in State of Missouri - Required within 60 Days Basic Life Support (BLS) - Required within 90 Days Travel Requirements Must be able to travel between various system facilities and off-site locations as needed. - Required Benefits Mosaic has a wide array of benefits to meet each employee’s individual needs.
