Description

• Conduct assessments for patients in both inpatient and outpatient environments and formulate tailor-made treatment plans
• Implement physical therapy treatments focused on enhancing mobility, strength, balance, and overall function
• Partner with nursing staff, doctors, case managers, and other professionals in rehabilitation to optimize patient outcomes
• Educate patients and their families on therapeutic goals, discharge procedures, and home exercise regimens
• Maintain thorough documentation of evaluations, treatments, and progress in line with regulatory and facility standards
• Engage in interdisciplinary team rounds and conferences to ensure seamless coordination of care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program

Certifications

• Must possess state licensure as a Physical Therapist
• CPR certification is preferred
• Specialty certifications (such as neurological or geriatric) are advantageous but not mandatory

Skills

• Prior experience is highly preferred
