Community Health Advocate — Hybrid Role

Point32Health is seeking a Community Health Worker to assist members in managing health goals in the community. The CHW collaborates with the Integrate Care Management team to improve knowledge, motivation, and adherence to care plans. The role includes coordinating safety net services, education, housing, healthcare access, and support for social determinants of health.
