As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and moreall working together to deliver personalized, high-quality care. In this role, you will work onsite at our Med Center and Boerne clinics located in the San Antonio area. You will report to our Associate Medical Director. Team-Based Care Model: Partner with a dedicated care team that supports patients' physical, emotional, and social wellness. More Time with Patients: See fewer patients per day and spend more time delivering meaningful care. We foster a welcoming, inclusive environment where teamwork and growth are prioritized. You will provide comprehensive care to seniors following clinical standards. You will collaborate daily with your care team to ensure coordinated, high-quality care. You will manage referrals, hospital/SNF coordination, DME, and home health services. You will document care accurately and efficiently with support from quality-based coders. You will participate in on-call rotation and support growth plans at your center. You will have passion for providing value-based care. You will have a Master's Degree in Nursing or completion of a PA program with board certification. You will have 2 or more years of experience in value-based care, primary care or Medicare Advantage settings. The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. 123,800 - $170,300 per yearThis job is eligible for a bonus incentive plan.
