We are seeking an RN Case Manager Care Coordinator to support patient care coordination, discharge planning, utilization review, and interdisciplinary collaboration to ensure high-quality patient outcomes throughout the continuum of care.

Responsibilities
• Perform comprehensive psychosocial and medical assessments
• Develop and coordinate individualized case management plans
• Collaborate with interdisciplinary teams on discharge planning
• Conduct utilization reviews and assess medical necessity
• Coordinate communication between physicians, patients, families, and outside agencies

Qualifications
• Minimum 3 years of recent acute care Case Management experience required
OR
• 3 years of experience in Med/Surg, Tele, Neuro, ICU, PCU, or ED

Candidates with home health or insurance case management backgrounds may also be considered if they have:
• 3 years total acute care experience
• At least 1 year of recent acute care experience within the last 5 years

Job Details
• Full-Time | Permanent
• Evening Shift: 12:00 PM – 8:30 PM EST
• Weekend Requirement: 1 weekend every 4 weeks
