Job Description & Requirements\n \n Specialty: Case Management \n Discipline: RN \n Duration: 13 weeks \n 36 hours per week \n Shift: 12 hours \n Employment Type: Travel \n \n Job Description & Requirements \n RN Case Manager \n StartDate: ASAP Pay Rate: $1800.00 - $2500.00 \n POSITION SUMMARY – RN Case Manager \n POSITION DUTIES – Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient’s medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning. \n MINIMUM REQUIRED QUALIFICATIONS – \n \n \n FL/compact RN license \n BLS \n 2 years minimum of acute care hospital experience in specialty \n Acute hospital experience in specialty during the past year with no more than a 90-day break \n Meditech & Midas experience \n \n \n PREFERRED QUALIFICATIONS – Complex discharge planning experience \n LENGTH OF ASSIGNMENT – 13 weeks \n SHIFT / HOURS PER WEEK – 8:30a - 5p. Every other weekend. \n SYSTEMS – Meditech, Midas \n START DATE – ASAP \n \n Facility Location \n \n A beautiful 150 acre Botanical Garden represents Florida’s extensive native and tropical plants and has amazed all those who go visit the garden.
