Wound Nurse/Infection Preventionist/LPN Job Description
The Wound Nurse/Infection Preventionist/LPN is a skilled healthcare professional responsible for providing specialized care in wound management and infection prevention within the healthcare setting. This role combines the expertise of an LPN with advanced knowledge in wound care and infection control practices. The individual will assess, plan, implement, and evaluate wound care treatments, as well as support infection prevention initiatives to reduce the risk of healthcare-associated infections (HAIs). The Wound Nurse/Infection Preventionist/LPN collaborates with interdisciplinary teams to ensure high-quality, evidence-based patient care.
Key Responsibilities
Wound Care Management
- Assess and document the condition of patients' wounds, including pressure ulcers, diabetic foot ulcers, surgical wounds, and other complex injuries.
- Develop individualized care plans for wound management, using best practices and evidence-based guidelines.
- Administer wound care treatments, including dressing changes, debridement, and application of topical treatments or advanced wound care products.
- Monitor and evaluate wound healing progress, adjusting care plans as necessary.
- Educate patients and their families on proper wound care techniques, lifestyle changes, and strategies to promote healing and prevent complications.
Infection Prevention
- Implement infection prevention protocols and provide guidance on best practices for maintaining a sterile environment in patient care settings.
- Conduct regular surveillance of healthcare-associated infections (HAIs) and track trends to identify potential outbreaks or areas for improvement.
- Provide infection prevention education to staff, patients, and families, focusing on proper hand hygiene, isolation procedures, and other preventive measures.
- Participate in infection prevention audits and ensure compliance with relevant guidelines and regulations.
- Assist in the development and revision of infection prevention policies and procedures based on current research and recommendations from health authorities (e.g., CDC, WHO).
Patient Education and Support
- Offer educational resources and support to patients, families, and staff regarding wound care, infection prevention, and healthy practices.
- Act as a resource for nursing staff regarding the management of complex wounds and infection control issues.
- Participate in care conferences to address wound care and infection control concerns for patients with complex health conditions.
Documentation and Reporting
- Maintain accurate and detailed documentation of wound care assessments, treatments, and patient progress.
- Document infection prevention activities, including surveillance data, audits, and educational sessions.
- Provide reports and data to nursing leadership and infection control committees to support quality improvement initiatives.
Collaboration and Communication
- Work closely with physicians, nurses, and other healthcare professionals to develop and implement comprehensive patient care plans.
- Communicate effectively with patients and families, ensuring they are well-informed about the care process and infection prevention strategies.
- Participate in interdisciplinary rounds and team meetings to provide expert input on wound care and infection prevention matters.
Qualifications
- Education & Licensing:
- Current, valid licensure as a Licensed Practical Nurse (LPN) in the state of Missouri.
- Certification in Wound Care (e.g., Wound Care Certification or CWCN) preferred or obtained within a reasonable time frame.
- Infection Control Certification (e.g., CIC) or completion of relevant infection control training preferred.
- Experience:
- Minimum of 2 years of clinical nursing experience, with a focus on wound care or infection control preferred.
- Prior experience in long-term care setting.
- Skills & Abilities:
- Strong knowledge of wound care principles, treatment modalities, and infection prevention practices.
- Ability to perform detailed wound assessments and administer advanced wound care treatments.
- Proficient in infection surveillance, reporting, and compliance with infection control standards.
- Excellent communication, organizational, and problem-solving skills.
- Ability to educate and support patients, families, and staff in complex care situations.
- Knowledge of current healthcare regulations and infection control guidelines (e.g., CDC, OSHA, state regulations).
