Job Title: Pediatric Advanced Practice Provider (APRN / PA-C) Care at Home Program

Location: Jacksonville, FL

Job Type: Full-Time M-F, 3 days are 8a-5p , and 2 days are 11a-7

Position Overview

We are seeking an experienced Pediatric Advanced Practice Provider (APRN or PA-C) to join an innovative home-based acute care initiative. Working on behalf of a premier pediatric health system, this role focuses on supporting pediatric patients as they transition from hospital units to high-quality care in their homes. You will collaborate closely with attending physicians to round on inpatient units, enroll eligible patients, and manage ongoing treatment plans through dedicated telehealth encounters.

Key Responsibilities

Patient Care & Telehealth: Provide high-touch, virtual follow-up care for pediatric patients transitioning from inpatient care to home.
Program Collaboration: Conduct inpatient rounding with physicians to identify, evaluate, and enroll eligible pediatric patients into the home care initiative.
Care Coordination: Partner with multidisciplinary teams and nursing staff to formulate care and discharge plans, assess learning needs, and provide tailored patient/family education.
Clinical Oversight: Utilize prescriptive authority (where credentialed) to monitor compliance with treatment plans and make timely referrals for additional resources.

Qualifications & Requirements

Advanced Practice Registered Nurse (APRN):

Active, unrestricted APRN license in Florida (or eligible compact state) + Florida DEA registration.
Pediatric Nurse Practitioner Certification (PNCB/NCC) with Acute Care certification.
Current AHA BLS and PALS certifications upon hire.

Physician Assistant (PA-C):

Active, unrestricted PA license in Florida + Florida DEA registration.
Current NCCPA certification.
Current AHA BLS and PALS certifications upon hire.

Experience:

3 5 years of Advanced Practice experience in critical care pediatric settings required.
