How will you make an impact & Requirements $10K Sign-on Bonus Field Nurse Practitioner - San Gabriel Valley & East Los Angeles, CA Region: West Covina, El Monte, Whittier, Pico Rivera, & East LA This is a field-based role requiring travel to various clinical sites and patient locations across our San Gabriel Valley (SGV) and East LA service areas. Primary Duties Provides a range of primary, acute and/or specialty healthcare services for patients in home or when appropriate virtual settings. In states that do not require collaborative agreements, functions as the primary care provider. Orders, interprets and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs. Provides health instruction and counseling, educating patients on disease prevention and healthy lifestyle choices. May work in collaboration with other Physicians, Nurses or other healthcare professionals to treat the needs of the patient(s) as part of an interdisciplinary care team. Formulates and documents care plan. Prescribes medication or other forms of treatment. Might participate in On-Call program, which may include after-hours, weekend and holiday calls and visitations. Service Areas San Gabriel Valley: Pasadena, West Covina, El Monte East LA: Whittier, Pico Rivera, Downey Requirements Requires an MS in Nursing Minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background. Requires a current, active, valid and unrestricted RN license and NP license in Nevada. Satisfactory completion of a Tuberculosis test is a requirement for this position. his position is bonus eligible based on individual and company performance.
