Social Worker - CSW, APSW, LCSW

As a social worker you will provide comprehensive support across various medical social work areas, including hospital inpatient and transitional care. This position serves as a critical resource within multidisciplinary teams, focusing on patient-centered care and collaboration to ensure optimal discharge planning and coordination of services. Key Responsibilities * Collaborate with multidisciplinary teams to assess and support the needs of the patients and their families * Coordinate patient discharge planning, ensuring all necessary resources and support systems are in place * Conduct biopsychosocial assessments to evaluate patients' needs and appropriate interventions

• Engage patients and families in discharge planning processes
• Facilitate communication and coordination with community partners and
