Licensed Practical Nurse (LPN) Aspire Health Partners – Orlando, Florida, United States About this position Full‑time & pool positions are available across Orlando and Sanford. Pay starts at $27 per hour, with shift differentials and increases based on experience and credentials. Integrated Care Nurse – providing integrated behavioral health, addiction medicine, and primary care services. We are seeking talented and compassionate Registered Nurses and Licensed Practical Nurses to join our growing team. Job Responsibilities Conduct comprehensive assessments of patients’ physical and mental health, substance use history, and social determinants of health. Develop individualized care plans based on patients’ needs and treatment goals, integrating behavioral health, addiction medicine, and primary care interventions. Administer medications, provide medication‑assisted treatment (MAT) for substance use disorders, and monitor patients’ responses to treatment. Collaborate with an interdisciplinary team—including physicians, therapists, social workers, and other healthcare professionals—to ensure coordinated and holistic care. Offer evidence‑based counseling and psychoeducation to patients and their families, focusing on mental health, addiction, and chronic disease management. Conduct regular follow‑up visits to monitor patients’ progress, provide ongoing support, and adjust treatment plans as needed. Maintain accurate and up‑to‑date medical records and documentation, adhering to regulatory guidelines and best practices. Actively participate in continuing education and professional development opportunities. Qualifications Valid nursing license (RN or LPN) in Florida. Level II background clearance (all positions require Level 2 clearance). Experience working in integrated healthcare settings, behavioral health, addiction medicine, and/or primary care. Strong understanding of mental health disorders, addiction treatment modalities, and physical health. Excellent assessment and critical thinking skills. Ability to work independently and collaboratively within an interdisciplinary team. Outstanding communication and interpersonal skills.
