- Conduct thorough evaluations for patients within both inpatient and outpatient environments, customizing individual treatment strategies
- Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality
- Work collaboratively with a diverse team of nurses, physicians, case managers, and rehabilitation specialists to elevate patient outcomes
- Guide patients and their families through therapeutic objectives, discharge plans, and exercise regimens for continued care at home
- Accurately document patient evaluations, treatment processes, and progress while adhering to organizational and regulatory standards
- Engage in team rounds and collaborative conferences ensuring seamless, coordinated care
- Doctorate in Physical Therapy (DPT) or a degree deemed equivalent from an accredited institution
- Current state licensure as a Physical Therapist
- CPR certification preferred
- Additional specialized certifications (e.g., in neurology or geriatrics) are advantageous but not mandatory
- Previous experience in physical therapy is highly preferred
- Ability to manage patients with various health complexities and mobility challenges
- Excellent communication and collaboration skills
- Proficient in using digital documentation and clinical management systems
