Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: 8H Days M-F Hours per Shift: 8 Experience: Minimum of 2 years in Inpatient Case Management License: State RN License or compact Certifications: BLS Certification, Preferred certification in Case Management Must-Have: Experience in hospital utilization management, proficiency in InterQual and MCG Description: Registered Nurse Case Managers will work collaboratively with the Care Team. They will complete comprehensive patient assessments to drive discharge planning. The role involves driving stay progression and ensuring effective transitions to the next level of care. Accurate and comprehensive documentation in the electronic health record system is required. The team will have primary unit assignments but may cover other units as needed. It is critical for candidates to have experience in hospital utilization management. The role requires the ability to work independently and follow through to completion without prompting. Candidates must be able to complete patient assessments and drive discharge planning effectively. Participation in weekend rotation is required. The unit may include a patient bed range of 20-36. There may be one or two RN Case Managers assigned to a unit, based on unit size. Business casual attire with a white lab coat is required, along with closed-toed shoes or designated scrubs. Training and orientation will include specific departmental orientation and electronic health record training. This position plays a key role in ensuring seamless patient care and discharge processes.
