Day shift, 5x8-Hour (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a skilled Home Health Physical Therapist to provide high-quality care to patients in the comfort of their homes. The ideal candidate will have a strong background in home health and will be capable of managing a maximum of 6 visits per day, adhering to a productivity standard of 30 points.

Responsibilities

• Conduct assessments and develop individualized treatment plans for patients.
• Implement therapeutic interventions and track patient progress.
• Educate patients and their families on home exercises and mobility techniques.
• Document patient care and outcomes using EPIC Remote Client and Rover applications.
• Maintain communication with interdisciplinary teams to ensure comprehensive care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience.
• BCLS certification required.
• OASIS experience highly preferred.
• Must have reliable transportation for daily patient visits, with occasional maximum travel of 60 miles.
