Responsibilities The Care Navigator (CN) plays a key role in enhancing client access to services through strategic community engagement, resource linkage, care coordination and enrollment of clients in the Families Rising programs. The CN builds and maintains relationships with community-based programs to ensure comprehensive support for clients. Core Responsibilities Maximize resource access for clients by becoming an expert on community resources and assist clients in accessing these resources, including prosocial activities, public benefits, housing, legal, education, vocational, and medical services, leading to improved client stability. Collaborate with community resources to support client needs. Contact clients to obtain any missing insurance details for clients in the Families Rising program and verify insurance in the systems. Maintain accurate billing information for all clients 21 years of age and younger. Complete weekly checks of client curfew and school compliance. Report to the clinical team on compliance and cover court appearances to report out on compliance when needed. Conduct intake with clients in their home or community through the completion of initial assessments and gather essential consent forms for new cases, ensuring timely case openings, completeness of information and client understanding of services to achieve a smooth start to treatment. Enter contacts and intakes with clients and collaterals into agency database in a timely manner in accordance with agency and stakeholder guidelines to ensure that all work is thoroughly documented, up-to-date and accurate. Network with stakeholders to assist in receiving referrals. Assist in the screening of new cases by speaking to incoming client and families to describe services and obtain their consent. Participate in group and individual supervision, model consultations, trainings, all staff meetings and community outreach events to strengthen professional development, skill growth, group cohesion and community visibility. Provide coverage for peers when needed to ensure uninterrupted services. Qualifications High School Diploma/ GED and one year relevant work in child welfare or social services or life experience or A Bachelor's degree in social work, psychology, or a related field is preferred. Experience or knowledge of providing service coordination, linkages, and/or referrals to community-based programs. A passion for serving children and families. Relevant experience in child welfare, social services, or a related field is preferred. Excellent writing, communication, interpersonal, problem-solving, time management and organizational skills. Ability to work independently and as part of a team. Proven self-management abilities including meeting deadlines, prioritizing multiple tasks efficiently, timely completion of documentation and maintaining accurate and up-to-date case records. Ability to work effectively with a diverse, multidisciplinary team as well as a diverse client population. Ability to receive feedback to professionally grow and/or improve and be flexible with programmatic needs and changes. Must be comfortable and able to travel for in-person meetings at Foundling offices. Demonstrated strong commitment to safety.
