Pharmacist

• Evaluate and verify medication orders for home infusion therapies, including anti-infectives, parenteral nutrition (TPN), biologics, chemotherapy, and other specialty medications.
• Perform or oversee sterile compounding activities while ensuring adherence to USP <797> , USP <800> , and all applicable regulatory requirements.
• Verify the accuracy of compounded medications, labeling, and supplies before dispensing and coordinating delivery.
• Assess patient-specific medication regimens and provide clinical recommendations, including dosing adjustments and therapy optimization when appropriate.
• Monitor laboratory values, treatment response, and medication safety, intervening as needed to support optimal clinical outcomes.
• Educate patients and caregivers on infusion medications, administration techniques, storage requirements, potential adverse effects, and safe handling practices.
• Work closely with physicians, nurses, case managers, and other healthcare professionals to coordinate individualized patient care plans.
• Maintain complete and accurate documentation in accordance with state, federal, accreditation, and organizational standards.
• Provide guidance and oversight to pharmacy technicians and other support staff involved in sterile compounding and pharmacy operations.
• Participate in after-hours, weekend, or on-call clinical support as required.

Required Qualifications

• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited institution.
• Current, unrestricted pharmacist license in the applicable state.
• Experience in sterile compounding, with home infusion, specialty pharmacy, or hospital pharmacy experience preferred.
• Strong understanding of USP <797> , USP <800> , aseptic technique, and sterile preparation best practices.
• Solid clinical knowledge of intravenous medications and infusion therapies.
• Excellent communication, organization, and problem-solving skills with the ability to work effectively in a multidisciplinary environment.

Preferred Experience

• Board certification (such as BCPS) is a plus.
• Previous experience supporting patients in a home infusion or specialty pharmacy setting.
• Proficiency with pharmacy management software and electronic health record (EHR) systems.

Work Setting

• Primarily based in a pharmacy cleanroom and office environment.
• May involve occasional direct interaction with patients or caregivers, either in person or remotely.
• Schedule may include evening, weekend, or rotating on-call responsibilities.

Physical Demands

• Ability to remain standing for extended periods while performing pharmacy duties.
• Fine motor skills necessary for sterile compounding and medication preparation.
• Ability to lift and move medication shipments and supplies weighing up to approximately 25 pounds.
