Case Manager RN

Parkridge Health Network is seeking a Case Manager RN to coordinate and advocate for patients across the continuum of care. In this role, you will assess clinical and psychosocial needs, develop and manage individualized care plans, and lead safe, timely discharge planning. You will collaborate closely with physicians, nurses, therapists, social workers, and community partners to optimize outcomes, reduce readmissions, and ensure appropriate use of resources.
