Job Summary The Registered Nurse (“RN”) is responsible for supporting the physical health, safety, and wellness needs of clients participating in Minnesota Adult & Teen Challenge programs. The RN provides nursing assessments, health screenings, medication oversight, care coordination, health education, and consultation to clients and staff while collaborating with interdisciplinary treatment teams to support recovery and treatment outcomes. This position promotes a safe, therapeutic, and recovery-oriented environment and ensures nursing services are delivered in compliance with professional nursing standards, organizational policies, and applicable regulatory requirements. To perform this job successfully, an individual must be able to perform each essential job duty satisfactorily. The essential functions include the following: Admission & Initial Assessment Review admission information, referrals, assessments, and health-related documentation to identify client health needs and potential risks. Conduct nursing assessments, health screenings, and medication reviews for new clients in accordance with program requirements. Identify actual or potential health concerns and coordinate appropriate referrals, follow-up care, and interventions. Collaborate with admissions, clinical staff, and program leadership regarding client medical and health-related needs. Identify clients presenting elevated medical, medication-related, withdrawal-related, or other healthcare risks and coordinate appropriate interventions, referrals, and follow-up care. Treatment Planning & Care Coordination Participate as a member of the multidisciplinary treatment team by providing nursing input into client care, treatment planning, and ongoing service coordination. Coordinate healthcare services, appointments, laboratory testing, referrals, and follow-up care as appropriate, collaborating with psychiatric nurse practitioners, prescribers, and interdisciplinary treatment teams to support continuity of care. Collaborate and communicate effectively with physicians, advanced practice providers, pharmacies, healthcare providers, clinical staff, recovery services staff, and program leadership to support continuity of care. Advocate for client health, safety, and access to appropriate healthcare services. Assist with discharge planning and coordination of healthcare services to support continuity of care following program completion or transfer. Collaborate with psychiatric nurse practitioners and other healthcare providers to support withdrawal management, psychiatric care, medication monitoring, and continuity of healthcare services. Ongoing Client Care & Monitoring Monitor client health status throughout participation in services and respond to medical concerns, referrals, and requests from clients and staff. Assess client responses to interventions and communicate observations, concerns, and recommendations to appropriate treatment team members. Monitor and communicate changes to care team in client physical, behavioral, and mental health status. Monitor withdrawal-related symptoms, medication concerns, and changes in client health status, and communicate significant findings to psychiatric nurse practitioners, prescribers, and appropriate treatment team members. Provide nursing services and support regarding client health and wellness needs, within the scope of practice established by the Minnesota Nurse Practice Act and professional nursing standards. Escalate complex medical, psychiatric, medication-related, or client safety concerns to the Supervisor, Nursing Services, medical provider or other leaders as needed. Respond to referrals and requests from staff regarding client medical, medication-related, and healthcare concerns. Medication Management Review, monitor, and address medication-related concerns, including medication reconciliation, client education, and communication with prescribing providers. Assist clients with medication administration, medication education, medication monitoring, and prescribed injections in accordance with provider orders, nursing standards, and organizational policies. Monitor medication adherence, effectiveness, potential side effects, and adverse reactions. Ensure proper storage, handling, inventory management, documentation, auditing, accountability, and destruction of medications in accordance with organizational policies and regulatory requirements. Review, document, and report medication incidents, variances, adverse reactions, and medication-related concerns. Participate in medication inventory reviews, audits, and controlled substance accountability processes as assigned. Assist with medication-related education and training for staff and clients as appropriate. Implement and monitor provider orders related to medications, withdrawal management, and healthcare services in accordance with organizational policies, nursing standards, and scope of practice requirements. Health Education & Support Provide health education and wellness support to clients regarding medication management, infection prevention, chronic disease management, healthy living practices, and healthcare navigation. Support clients in understanding and managing their physical health needs in a manner that promotes recovery, personal responsibility, and overall wellness. Provide consultation, education, and guidance to staff regarding health-related concerns, medication management practices, communicable disease prevention, and client wellness needs. Safety, Infection Prevention & Emergency Response Monitor infection prevention practices and assist with implementation of communicable disease response protocols, exposure management, and public health reporting requirements as applicable. Identify and report safety concerns, incidents, health risks, and unusual occurrences to appropriate leadership in a timely manner. Respond appropriately to medical emergencies and assist with emergency response procedures in accordance with organizational policies and nursing standards. Documentation, Compliance & Quality Maintain accurate, timely, and complete documentation of nursing assessments, client interactions, medication-related activities, referrals, laboratory results, and other healthcare information within the electronic health record. Complete required documentation within established timelines and in accordance with professional nursing standards, Minnesota Statutes, organizational policies, and regulatory requirements. Maintain compliance with all applicable nursing practice standards, ethical requirements, licensure obligations, and organizational expectations. Support compliance with licensing, accreditation, and regulatory requirements, including audits, inspections, corrective action plans, and quality assurance initiatives. Participate in meetings, trainings, supervision, quality improvement activities, and continuous improvement initiatives as required. Provide recommendations regarding nursing practices, healthcare processes, policies, and procedures to support quality client care and continuous improvement efforts. Follow all program policies, guidelines, safety procedures and organizational standards. Maintain compliance with all required MnTC and DHS related training, completing assigned courses in a timely manner. Assist with maintaining appropriate inventory levels of medical, first aid, and healthcare-related supplies. Required Skills & Experience (Minimum Qualifications): Current Registered Nurse (RN) license, unrestricted and in good standing in the State of Minnesota. Must maintain licensure in accordance with the Minnesota Nurse Practice Act and Minnesota Board of Nursing requirements. Minimum five (5) years of previous nursing experience. Previous experience in substance use disorder treatment, behavioral health, community health, residential care, correctional healthcare or recovery services preferred. Comfort and compassion in working with individuals facing life-controlling challenges, including substance use disorders and mental health concerns. Strong clinical assessment, critical thinking and care coordination skills. Excellent written, verbal, and interpersonal communication skills. Proficiency with electronic health records, Microsoft Office applications, and other technology systems. Current Basic Life Support (BLS) certification or ability to obtain within three months of hire. Ability to effectively communicate with clients, families, healthcare providers, pharmacies, and interdisciplinary team members. Ability to exercise sound clinical judgment and prioritize nursing interventions in a fast-paced environment. Knowledge of substance use disorders, withdrawal management principles, co-occurring conditions, and recovery-oriented care practices. Strong organizational, time management, and attention-to-detail skills. Valid driver’s license and a clean driving record that meets MnTC’s insurance requirements. Must successfully complete a Minnesota Department of Human Services (DHS) background study and maintain eligibility for all DHS background studies, clearances, and requirements applicable to assigned services and programs prior to providing direct care or client services. Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Prolonged periods of sitting, standing, walking, bending, reaching, and working on a computer. Ability to occasionally lift and carry up to 30 pounds. Ability to travel between program locations, healthcare facilities, and community resources as needed. Ability to respond appropriately during medical emergencies and other urgent situations. Regular on-site attendance required to support client care, interdisciplinary collaboration, and operational needs. This position is not eligible to work remotely.
