About the Position

Job Title: Physical Therapist Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 16 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 1 year; first timers accepted License: Required GA PT License or Compact PT Certifications: BLS Must-Have: Experience with home health, geriatrics, strokes, and orthopedics. Description: Provide physical therapy services in a home health setting. Work with adult and geriatric patient populations. Utilize Home Care Home Base (HCHB) for tracking visits and documentation. Productivity expectation is approximately 25 units for full-time clinicians with 7 visits per day. Must complete an online HCHB course if lacking experience with the system. Mileage reimbursement is provided per IRS standards. Scrub color is not specified; any color is acceptable. No weekend shifts required. Floating may occur within scope of practice as needed. Expected to work one major holiday during the assignment period; potentially two if three major holidays fall within the assignment. Compliance with onboarding requirements is necessary to start on time. Clinical screening will occur to determine fit prior to final decision. Must be fully compliant to avoid delays. Required skills include assessment, adaptive devices, gait training, discharge planning, and more.
