You will be part of a multidisciplinary team that champions integrated healthcare, allowing you to collaborate with healthcare providers, specialists, and support staff remotely or when travel to our various sites is necessary. In this setting, effective communication and teamwork will be instrumental to your success as a Licensed Clinical Social Worker.

Key Responsibilities\n

As a Licensed Clinical Social Worker at CHDC, your responsibilities will include but are not limited to:

• Delivering clinical behavioral health services while adhering to the organization's mission and the highest quality standards, ensuring informed patient decisions and satisfaction.

• Guiding mental health consumers through the complexities of physical and behavioral healthcare systems to facilitate integrated care.
\n
• Providing education on available resources and services, fostering individual motivation to engage in disease prevention, wellness initiatives, and overall improved health outcomes.
\n
• Actively participating in multi-disciplinary treatment team meetings, contributing to holistic treatment planning and care delivery.
\n
• Ensuring prompt reporting of any crisis situations to the Medical Director and immediate communication of psychiatric or medical crises to the Chief Medical Officer (CMO).
\n
• Completing thorough documentation of patient interactions in clinical records and other areas as directed to maintain compliance and facilitate continuity of care.
\n
• Engaging in the organization's Quality Improvement program, attending all necessary meetings and training sessions.
\n
• Working as part of the integrated healthcare team to apply the model of integrated Primary Care Behavioral Health effectively.
\n
• Administering evidence-based screening tools for assessing patient healthcare needs and documenting these findings accurately.
\n
• Collaborating with the Center of Excellence Team in managing patients with Opioid Use Disorder, providing supportive and informed guidance through their recovery journey.

Patient Population / Client Focus\n

In this role, you will primarily focus on serving individuals grappling with varying degrees of mental health challenges and substance use disorders. Your work will directly impact vulnerable populations, enhancing their access to necessary resources, strengthening their resilience through integrated care, and promoting recovery-oriented approaches in both the Detox Center and beyond.

Qualifications\n

The successful candidate will meet the following qualifications:

• A master’s degree in social work is mandatory, and a current Licensed Clinical Social Worker (LCSW) credential is required.

• A minimum of 2 to 3 years of relevant experience in social work, particularly within healthcare settings, is essential.

• Proficiency in collaborating with diverse healthcare professionals, social service organizations, and clients requiring multi-faceted support.

• This position may necessitate travel to various CHDC sites for meetings, training sessions, and outreach functions; flexibility and mobility are expected.
