Physical Therapist

Acts is currently seeking qualified candidates for our Physical Therapist position. In this role, you will be responsible for providing physical therapy services to residents, including evaluations, planning individual treatment programs and goals, actual treatments, discharge planning, and performing other related duties as required. Responsibilities In-house program within a welcoming, slower-paced community environment that delivers holistic care with a strong focus on prevention State-of-the-art rehab gyms and equipment; opportunity to build relationships by following residents across the full continuum of care, including inpatient, home health, outpatient, and fitness services Clinically driven, patient-centered treatment decisions guide all programs and care plans Positive leadership philosophy that gives therapists opportunities to provide input into programming, systems, and creative idea sharing Provide physical therapy services to residents, including evaluations and planning, treatments, discharge planning, and related duties Qualifications Bachelor\'s degree in Physical Therapy Valid Physical Therapist license or eligibility for license in the state(s) where services are rendered Experience in a skilled nursing and/or outpatient rehab environment (preferred) Benefits and About Acts Team members are eligible for a generous benefit package including health benefits (medical, prescription, dental and vision), flexible spending accounts, life insurance, disability programs, 401(k) plan (with 4% company match after one year of employment), paid time off and holidays, and more.
