RN Case Manager

Conduct initial client assessments and continuously refine care plans Provide hands-on nursing care and patient education Maintain open lines of communication with patients, families, and the healthcare team Manage patient care coordination for your caseload Participate in interdisciplinary team meetings to ensure comprehensive care planning Make patients' final days, weeks, and months as comfortable as possible Work closely with the company's owners Contribute to internal growth and clinical advancement Requirements Licensed as a Registered Nurse / RN in the state you work in Proficiency with Electronic Medical Records (EMR) software Excellent planning, organizing, and communication skills Ability to solve problems autonomously while maintaining high standards Valid Driver's License Reliable vehicle with current insurance Core Competencies Demonstrates expertise in patient care coordination, hands-on nursing, and effective communication with patients and healthcare teams. Proficient in utilizing Electronic Medical Records (EMR) software to enhance care planning and patient education.
