Case Manager

Overview Help individuals with disabilities live more independently and confidently. As a Case Manager, you'll build meaningful relationships, coordinate services, and connect clients with the resources and support they need to achieve their goals and enhance their quality of life. ServiceSource is a champion for people with disabilities and focuses on innovation, change, and community strength. Responsibilities Use a holistic approach to develop a customized care plan related to health, wellness, finances, living situation, activities of daily living, assistive technology, and life transitions. Continuously assess changes in status and support needs and update the care plan as needed. Support clients and families along the continuum of care; make referrals and linkages with community resources. Build and maintain long-term relationships with clients, family members, and other stakeholders (e.g., medical teams, paid and natural supports, legal guardians). Provide case management through development of a care plan and a system for storage and sharing of records; provide updates to team members as necessary. Identify and provide assistive technology supports to maintain independent living; include installation, training, monitoring, coordination of assessments/training, and related documentation to obtain assistive technology. Provide referrals and coordinate services with community resources. Coordinate medical supports: arrange transportation, attend medical appointments, and follow-up as needed; assist with medication management and refills. Develop an emergency response plan and provide backup emergency response support. Identify and provide in-home supports (e.g., companion/drop-in*, check-ins), meal preparation, cleaning, and home maintenance coordination. Identify and provide social supports: plan and participate in leisure and recreational activities; arrange transportation to these activities. Identify and coordinate other transportation needs; provide transportation as identified in the care plan. Identify and provide financial and decision-making supports: budgeting, trustee/representative payee services, coordinating long-term care and insurance options, conservatorship, guardianship, or power of attorney. Perform other responsibilities as assigned. Qualifications Education, Experience, and Certification: High school diploma or GED required. Bachelor’s degree in social work, recreation therapy, counseling, or other human services related field required. Valid driver’s license for work-related travel and eligible for coverage with ServiceSource’s insurance carrier required. Experience with case management and related duties required. Master’s degree in social work, recreation therapy, counseling, or related field preferred. Experience with install/setup of assistive technology preferred (smart home tech). Experience: 2–5 years in working with older adults or adults with disabilities preferred; 2–5 years in case management/plan development/coordination preferred. Knowledge, Skills, and Abilities: Proficiency in Microsoft Office; strong verbal and written communication; excellent organizational skills; detail-oriented; committed to social inclusion and strengths-based approach; knowledge of continuum of care, transition planning, Medicaid waiver requirements, WaMS, and assistive technology devices/services. Ability to establish positive working relationships with client systems; experience writing individual support plans and progress notes; ability to coordinate and document care plans and discharges. Physical Demands The physical demands described are representative of those required to perform the essential functions. While performing duties, the employee is regularly required to walk, sit, and stand; reach with hands and arms; and talk, see, and hear. May lift or move durable medical equipment, supplies, or groceries up to 50 pounds. May assist participants with balance and transferring to and from a wheelchair or vehicle. Specific vision abilities include close vision, distance vision, depth perception, and ability to adjust focus. Walking up and down stairs may be required. Reasonable accommodations may be made for individuals with disabilities. Work Environment The work environment is typically quiet or moderate in noise level; however, loud noise may be encountered. Duties may be performed during inclement weather and in various outdoor temperatures.
