Speech Language Pathologist – SLP Location: Boonsboro, Maryland (near Hagerstown) • Full Time • Fahrney Keedy • Salary: $46.00 - $56.00 per hour Benefits: Full Benefits, 401(k), Vacation and Sick Time. Summary Provide practical Speech Language Pathologist skills to residents following Fahrney‑Keedy’s policies and procedures. Maximize the communicative and swallowing competence of each resident. Provide comprehensive diagnostic, consultative, and treatment services for residents with hearing, language, speech, voice, cognitive and swallowing disorders. Communicate effectively with residents, family members, visitors, staff, and other professionals. Resident Care Duties & Responsibilities Provide speech therapy screenings, evaluations, and treatments according to your scope of practice to residents. Obtain informed consent from the resident and/or Power of Attorney. Perform all assigned tasks in accordance with established policies and procedures. Render initial evaluations for newly referred residents and ongoing speech treatment programs; provide re‑evaluations, guidance, and counseling to residents, families, and caretakers regarding Speech Therapy needs. Participate in the development, coordination, and implementation of resident care and discharge plans. Evaluate resident’s progress periodically and discharge from SLP services as indicated. Follow work assignments and schedules in completing and performing tasks. Complete and submit evaluations by end‑of‑treatment day, summarizing the resident’s needs, treatment plan, diagnoses, goals, prognosis, results, and discharge planning. Maintain weekly clinical progress notes, daily treatment encounter notes, and rehabilitation discharge programs for each resident via Optima. Treat residents with current techniques according to physician orders and ethical standards. Assist in ensuring therapy services meet the resident care plan and State Practice Act requirements. Cooperate with inter‑departmental staff to ensure therapy services address resident needs. Create and maintain a warm, personal, positive environment throughout the unit and shift. Report all complaints and grievances made by a resident immediately. Notify the facility of any lateness or absence at least two hours before shift start. Report work‑related injuries, illnesses, or exposure to hazardous materials to the supervisor. Adhere to the minimum weekly productivity standard of 85 %. Complete additional tasks as outlined in the Evaluating Therapist Task List. Maintain confidentiality of resident information and protect login credentials for facility information systems. Report any unauthorized access attempts to the facility’s information system. Log off or set screensaver automatically on unattended workstations per policy. Keep call buttons reachable to each resident. Notify the Director of Therapy or Director of Nursing of any resident missing from the facility. Follow safety precautions and maintain equipment cleanliness. Report all hazardous conditions, safety violations, communicable diseases, infection control matters, and isolation precautions to appropriate authorities. Use safety equipment and supplies appropriately; report defective equipment. Practice hand hygiene with sanitizer or washing as required. Working Environment Work throughout the facility. Subject to frequent interruptions. Encounter hostile or emotionally upset residents, family members, visitors, etc. May work beyond normal hours, on weekends and holidays. Subject to call‑back during emergency conditions. Exposure to hazardous chemicals and infectious waste diseases. Exposure to potential injuries from falls, equipment, odors, dust, disinfectants, and other air contaminants. Resident Rights Maintain confidentiality of all resident care information, including protected health information. Treat all residents fairly with kindness, dignity, and respect. Ensure nursing care is provided in privacy. Knock and wait for permission before entering resident rooms. Report resident grievances and complaints to the Delegating RN. Report allegations of resident abuse or misappropriation of property to the Abuse Coordinator. Honor resident’s refusal of treatment; report to supervisor. Specific Requirements Maintain current licensure and certification. Maintain CPR and First Aid certification. Attend continuing education as required by law for licensure maintenance. Participate in program development, in‑service training, and educational programs related to Speech Language Pathology. Provide education to the Rehabilitation Department following CEU events. Demonstrate knowledge and skills to provide age‑appropriate care. Be a supportive team member and exhibit teamwork. Make independent decisions when warranted. Interact tactfully with staff, residents, family, visitors, agencies, and the public. Work harmoniously with other staff. Have patience, tact, a cheerful disposition, and enthusiasm for handling difficult residents. Seek out new methods and incorporate them into practice. Communicate resident condition information effectively. Not pose a direct threat to the health and safety of others. Education Master’s Degree in Speech Language Pathology and certification of Clinical Competence. Licensure in applicable state. Physical & Sensory Requirements Be able to move intermittently throughout the workday. Speak and write English understandably. Handle mental and emotional stress inherent to the position. See and hear, or use prosthetics that provide adequate functioning. Function independently with flexibility, personal integrity, and effective collaboration. Meet general health requirements, including medical and physical examinations. Work with ill, disabled, elderly, emotionally upset, and hostile individuals.
