Medical Temporaries, Inc. is currently seeking an experienced ***Case Manager III-Outreach***.

• Minimum of two years of experience required.

This is a FULL TIME/TEMP position requiring availability between the hours of 8AM-4:30PM, Monday through Friday, located in Norfolk, VA (23502).

Responsibilities :

A Day in the life of a Case Manager III - Outreach:

As a Case Manager III - Outreach you will provide street and site outreach to homeless individuals and families. Assist consumers in accessing needed services to include housing, medical, social, educational, vocational, and other supports necessary to meeting basic needs. Completes documentation in accordance with agency guidelines.

Requirements for the Case Manager III - Outreach :

• Required : Bachelor's Degree in human services or a related field
• Required : Case Management experience
• Required : BLS
• Required : Possession of a valid driver's license and an acceptable driving record as reported by the DMV
• Required : Ability to pass Background Check and Drug Screen
• Required : Must be reliable and a quick learner and have ability to multi-task with accuracy in a very busy environment
• Required : Excellent written and verbal communication skills
• Required: Certificate of completion for peer specialist training

Preferred Qualifications for the Case Manager III - Outreach :

• Preferred : Mental Health experience
• Preferred : Outreach Case Management experience
• Preferred : Peer Recovery experience

Knowledge:
• State Licensure Requirements
• Consumer Rights Regulations
• Case Management Components
• Available community resources

Skills:
• Linking, Coordinating, Monitoring, Assessing, Evaluating
• Crisis prevention, intervention, and management
• Consumer Education
• Maintenance of consumer records in accordance with all regulatory standards
including timely documentation and individualized and fully integrated service plans
• Management of caseload between 30-50 consumers
• Liaison with community resources
• Advocacy
• Boundary and Limit setting
• Basic skills in Microsoft Word computer applications

Abilities:
• Demonstrate positive interpersonal communication and effective listening skills

• Advanced organizational ability
• Function as a member of an interdisciplinary team
• Work independently with minimal supervision
• Establish and maintain effective working relationships with colleagues, consumers,
and families
• Demonstrate adherence to strict confidentiality and ethical standards
• Exercise sound judgment
• Manage multiple work priorities
• Provide services in the field
• Project a professional image
