We are seeking a full-time Physical Therapist to join our team in Milwaukee, Wisconsin. In this role, you will report to the Clinical Manager and assist patients in regaining their strength, mobility, and independence from the comfort of their homes. By providing tailored care that emphasizes functional improvement, you will empower individuals to overcome challenges and enhance their quality of life.

Key Responsibilities

• Administer prescribed physical therapy treatments to patients in their homes, aiming for optimal physical function.
• Evaluate and document patients' physical capabilities to aid physician assessments and inform treatment objectives.
• Design and implement rehabilitation programs aligned with the physician's treatment plan and the patient's goals.
• Provide clear written care instructions to Physical Therapy Assistants (PTAs) and Home Health Aides, overseeing their performance and documentation.
• Thoroughly document patient observations, interventions, and evaluations in a timely manner.
• Communicate patient progress to the physician, Clinical Manager, care team, patient, and family, while submitting all necessary evaluations, plans, progress notes, and discharge summaries.

Qualifications

• Required: Degree from an accredited Physical Therapy Program recognized by the APTA.
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience working collaboratively with a healthcare team.
• Preferred: At least one year of experience as a Physical Therapist.

Compensation and Benefits

The pay range for this position is:

• $59.00 - $83.00 per visit/unit
• $93,000 - $128,000 annually as base pay

Working Conditions

This position is primarily remote, but occasional travel to our offices for training or meetings may be necessary. As part of our driver safety program, you will need a valid state driver's license and must maintain personal vehicle liability insurance that meets state minimum requirements.
