About the Role:

As an M-F Nurse, you will be responsible for providing direct patient care to our residents, including administering medications, wound care, and assisting with activities of daily living. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring our residents receive the best possible care.

Responsibilities:

• Provide direct patient care to our residents, including administering medications, wound care, and assisting with activities of daily living
• Develop and implement individualized care plans in collaboration with our interdisciplinary team
• Monitor and record vital signs, report changes to the healthcare team, and update care plans accordingly
• Assist with resident assessments, including identifying and addressing potential complications
• Collaborate with other departments, such as rehabilitation and social services, to ensure seamless care transitions
• Participate in quality improvement initiatives to enhance the overall quality of care
• Maintain accurate and up-to-date records, including medication administration and care notes

Requirements:

• Must be a registered nurse (RN) or licensed practical nurse (LPN)
