Home Care Physical Therapist

Overview Job Title: Home Health Physical Therapist - PT Location: Lynchburg, VA (and surrounding areas) Schedule: FT / PRN (Flexible Scheduling Available) Pay: Competitive per-visit rate (varies by visit type) Responsibilities Provide home-based physical therapy to patients recovering from illness or injury Conduct initial assessments and develop individualized treatment plans Monitor patient progress and modify care plans as needed Assist patients with exercises and hands-on therapy to improve mobility and manage pain Educate patients on their plan of care, therapy goals, and self-care techniques Coach family members on how to safely assist with care and exercises Collaborate with interdisciplinary team members (RN, LPN, CNA, OT, SLP) Maintain accurate and timely documentation Qualifications Graduate of an accredited Physical Therapist program Active PT license in the state of Virginia Minimum of 2 years of physical therapy experience (home health preferred) CPR certification and TB baseline screening Strong clinical judgment and critical thinking skills Excellent communication and interpersonal abilities Ability to work independently in a home setting Willingness to travel throughout the service area Benefits / Why Join Us Flexible scheduling One-on-one patient care Autonomy and work-life balance Supportive and collaborative team environment Online training and CEU opportunities Tuition discounts through Rasmussen University Opportunity to make a meaningful impact in patients’ lives #J-18808-Ljbffr
