Job Summary:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Beavercreek Health and Rehab, a premier care community in Beavercreek, Ohio. Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and other treatments as prescribed by physicians

• Conduct routine vital sign checks and document patient information accurately

• Develop and implement individualized care plans to meet the unique needs of each resident

• Collaborate with other healthcare professionals, including physicians, physical therapists, and occupational therapists, to ensure comprehensive care

• Communicate effectively with residents, families, and staff to ensure open and transparent care

• Maintain accurate and up-to-date medical records and reports

• Participate in quality improvement initiatives and maintain high standards of care

• Support and promote a positive and engaging environment for residents and staff
