Registered Nurse

Transitions Care is seeking a Registered Nurse to complete hospice admissions and initial assessments for patients with serious or terminal illness. In this field-based role, you will evaluate eligibility, gather histories, perform comprehensive nursing assessments, and initiate individualized plans of care. You’ll educate patients and families on hospice philosophy, symptom management, and available support, coordinating closely with physicians, social workers, chaplains, and aides. This role offers meaningful patient- and family-centered work, strong teamwork, and opportunities to grow within a mission-driven hospice organization.
