2. Coordinate at least 80 mobile outreach and education visits at 40 or more community sites during the grant period.
3. Coordinate approximately two mobile outreach and education trips per week throughout San Joaquin, Stanislaus, and Tuolumne counties.
4. Establish and maintain professional relationships with tribes, Native-serving organizations, schools, treatment providers, behavioral-health agencies, social-service organizations, and community partners.
5. Coordinate recovery education workshops for Native men, women, boys, and girls.
6. Coordinate hybrid community town halls featuring professionals knowledgeable about opioid use disorder, stimulant use disorder, polysubstance use, treatment, and recovery.
7. Assign outreach activities and provide day-to-day directions and support to outreach staff.
8. Provide or coordinate education regarding opioid use, stimulant use, polysubstance use, overdose prevention, naloxone, medication safety, safe drug disposal, treatment options, and recovery resources.
9. Coordinate the assembly and distribution of medication lock boxes, drug-deactivation pouches, educational materials, and other approved harm-reduction education supplies.
10. Ensure naloxone and other program materials are stored, transported, distributed, and documented according to organizational and funding requirements.
11. Provide respectful, nonjudgmental engagement and culturally appropriate education to individuals who currently use substances, individuals in recovery, families, and community members.
12. Coordinate warm referrals to substance-use treatment, MAT/MOUD providers, behavioral-health services, recovery support, and other community resources.
13. Participate in the San Joaquin Opioid Safety Coalition, CCUIH activities, Unite California, and other approved community partnerships.
14. Assist with developing and maintaining memoranda of understanding and referral relationships with treatment and community partners.
15. Help enroll additional referring agencies into the Unite California network.
16. Coordinate with staff and the media contractor on culturally and linguistically appropriate anti-stigma messaging, storytelling, presentations, public service announcements, and social-media content.
17. Ensure that campaign materials receive all required organizational and funder approvals before publication or distribution.
18. Maintain complete and accurate program documentation, including outreach logs, participant counts, sign-in sheets, demographic information, supply inventories, referral logs, surveys, evaluations, partnership records, and meeting minutes.
19. Protect confidential and sensitive participant information and follow all organizational privacy policies.
20. Monitor progress toward grant goals and promptly report concerns, delays, or unmet targets to the Program Director.
21. Participate in weekly program meetings, staff training, grant monitoring, site visits, and evaluation activities.
22. Assist the Program Director and Grant Manager with narrative reports, data collection, invoices, and the cumulative final report.
23. Represent Native Directions and Native C.O.R.E. professionally at outreach events, meetings, coalitions, trainings, and community functions.
