Outpatient Physical Therapist

This is a 13-week contract position.

5x8.0 shift schedule, with hours starting at 8:00 AM.

The Outpatient Physical Therapist is responsible for evaluating all assigned patients and developing, implementing, and modifying treatment plans in accordance with patient needs, goals, and physician direction. This role involves the delegation and direction of all discipline-specific therapy services for assigned patients and ensures the design of functional treatment programs for effective community reintegration. The therapist also serves as an educator for patients, families, students, coworkers, and themselves while performing other professional duties associated with patient care and therapy service development.

• Complete comprehensive evaluations of assigned patients.
• Develop appropriate treatment plans and goals to guide patients towards discharge within an established length of stay.
• Document medical records in a clear, concise, accurate, and timely manner following state, federal, and organizational guidelines.
• Provide care under signed, valid physician orders and within insurance approval as necessary.
• Participate in student program supervision and development of learning objectives.
• Create and actively pursue an individualized professional development plan.
• Maintain professional licensure and adhere to ethical standards of conduct.
• Engage in performance improvement initiatives to enhance service quality.

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• CPR Certification.
