Registered Nurse – Case Management

Schedule: Day Shift: 7:30 AM - 4:30 PM, Weekend Rotations

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40 per week

Requirements:

• Montana Licensure (Registered Nurse)
• American Heart Association BLS
• Minimum of 2 years of nursing experience
• Must have utilization review experience
• Preferred: Bachelor's Degree in Nursing
• 2 years of acute care hospital experience as a case manager

Roles & Responsibilities:

• Collaborate with physicians and multidisciplinary team members to develop and implement care plans for assigned patients.
• Conduct comprehensive assessments to determine eligibility for case management services based on clinical judgment and identified triggers.
• Monitor patient progress towards desired outcomes and adjust care plans as necessary.
• Engage with patients, caregivers, and other healthcare professionals to ensure continuity of care and address barriers to effective patient management.
• Maintain an ongoing caseload with regular outreach and management to ensure patient needs are met.
• Promote integration of services, including behavioral health and long-term support services, to enhance the continuity of care.

Compliance / Notes:

• The facility reserves the right to cancel three (3) shifts per 13-week contract in the event of low census or other unforeseen circumstances.
• Candidate must live at least 90 miles away from the facility to be considered.
