Responsibilities The Case Management Associate educates patients, families, and staff on the benefits and availability of home health care, hospice, skilled nursing facilities, nursing homes, rehabilitation services, and home infusion services. Coordinates care in homebound service areas and adheres to National Patient Safety Goals as appropriate. After the Social Worker or RN Case Manager complete recommendations, the Associate completes the plan for discharge, coordinates documentation, arranges transportation, and performs related clerical duties and utilization management. Case Management Responsibilities Identify potential candidates who could benefit from homebound services. Educate the patient/family prior to discharge regarding available services. Facilitate effective communication about qualifying criteria for various post‑acute care venues. Document referrals in the medical record according to departmental policy. Serve as a resource to the Physician, Case Manager, Social Worker and interdisciplinary team regarding post‑acute care needs. Educate patients on standards of quality for patient satisfaction measures. Case Management Only Maintain constant communication between patient, family, RN Case Manager and the Social Work Team regarding pending discharge arrangements. Communicate appropriate medical records to receiving agency/facility and to the appropriate payer. INTEGRIS Decisions Only Lead age‑ and developmentally appropriate patient education groups addressing the emotional, physical and environmental needs of Behavioral Health and/or Addiction Recovery patients. Participate in writing treatment plans with program staff and physicians. Assist in utilization review by contacting patients’ insurance companies to provide clinical information for authorization. Serve as a patient advocate for patient needs and those involved with the patient. Complete or facilitate the completion of emergency detention paperwork and ensure its distribution to legally designated individuals. Process orders/refills or laboratory orders for the prescribing physician. Monitor patients’ medications and provide education on use and side effects. Monitor vital signs as indicated by physician orders or if medically necessary. Provide referral options for patients who are assessed but do not enter the program. Complete a comprehensive initial mental health assessment of patients’ clinical, psychological and financial needs using available resources. Attend and participate in the treatment team. Qualifications Required Qualifications Bachelor’s degree in a health‑related field or equivalent business degree with two additional years of healthcare experience. Associate degree with two years experience in a variety of clinical settings. Licensed Practical Nurse (LPN) with three years of nursing experience. Six years of directly related experience in lieu of a degree. Current LPN licensure in Oklahoma or a current multistate license from a Nurse Licensure Compact member state. BLS (Basic Life Support) certification issued by the American Red Cross or American Heart Association within 30 days of hire. Preferred Qualifications Two years of experience in home health.
