Your responsibilities include:• Developing and maintaining individualized client care plans• Completing supervisory visits and in-home assessments• Monitoring client condition and recommending care adjustments• Providing hands-on caregiver training• Ensuring caregiver competency with delegated tasks• Reassessing clients every 90 days or sooner if needed• Documenting changes in condition and updating care plans• Communicating clearly with clients, families, and the care teamABOUT GRISWOLD:With over 200 locations across 31 states, Griswold is one of the country’s top home care companies, providing and referring professional, compassionate Caregivers around the clock.
