HEDIS Coordinator

The HEDIS Coordinator will manage patient panel oversight, track key empanelment metrics, and conduct staff training. This role supports scheduling, communication, and care coordination. It requires a detail-oriented professional with strong interpersonal skills and a commitment to delivering patient-centered care. Manage and coordinate patient panels, including scheduling appointments, follow- ups, and tracking patient engagement. Assist in onboarding new patients, ensuring accurate collection and maintenance of medical records. Communicate with patients about their health needs, upcoming appointments, and personalized care plans. Monitor patient data and clinical outcomes to identify areas for improvement in care delivery. Handle administrative duties such as insurance verification, eligibility and patient registration. Collaborate with clinical staff to gather data and verify the accuracy of patient records and documentation. Stay informed about changes in HEDIS requirements and healthcare regulations to ensure ongoing compliance. Facilitate training and education for staff on HEDIS measures and quality improvement initiatives. Assisting members in scheduling appointments and completing preventive health services in accordance with HEDIS/STARS measures. Stay up to date on healthcare quality regulations and standards to ensure compliance and adherence to best practices. Conduct outreach for patients with chronic conditions to ensure they receive necessary services. Contribute to a positive patient experience by providing courteous telephone interactions and appropriately referring cases to clinical staff when necessary. Review patient medical charts to verify the provision of necessary services and identify areas for quality improvement. Document and notify relevant personnel of any data inconsistencies discovered during patient record reviews. Perform all job functions in accordance with federal, state, and local laws, as well as company policies and procedures. Fulfill all duties of a Certified Medical Assistant (CMA), including rooming patients, taking vitals, collecting medical histories, administering injections, performing The pay range for this position at commencement of employment is expected to be between $23.00 and $24.00/hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus, restricted stock units, and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered. Certification/Licensure: Certified Medical Assistant (CMA) Certification. Certification/Licensure: CPR and first aid certification Experience: 1 year of healthcare experience, preferred. HEDIS measure management, preferred Skill: Ability to utilize computers, computer software/applications, and Microsoft Office applications (Excel, Outlook, PowerPoint, Word, etc.) with basic - intermediate proficiency. Skills: Ability to communicate effectively with individuals within all levels of the organization, providers, patients, patient family members, and others.
