Position Overview:

We're seeking a dedicated Wound Care Nurse to support our patients' healing journeys. As the Wound Nurse, you'll not only provide care but also educate patients and their families on preventive measures and healthy practices. The ideal candidate will exhibit responsibility, excellent training, and the ability to deliver outstanding nursing care with minimal supervision. Your role as a Wound Nurse (LPN) will include adherence to health and safety protocols to ensure consistent, high-quality nursing care. Your mission is to enhance patients' well-being through your exceptional nursing expertise.  

Responsibilities:

As the Wound Care Nurse (LPN), you'll:

• Assess, stage, treat, and monitor all wounds, including pressure injuries, surgical wounds, and chronic ulcers, using evidence-based protocols. 

• Develop and oversee individualized wound care plans, ensuring timely treatments, proper dressing changes, and early identification of infection or decline. 

• Lead pressure-injury prevention efforts through skin assessments, risk evaluations, staff education, and monitoring of repositioning and support surface use. 

• Maintain accurate wound documentation to support care planning, MDS accuracy, reimbursement, and regulatory compliance
