Department Personal Care Homes Reports to Residential Manager Position Purpose Responsible for providing a supportive home‑like environment for the Villagers residing in his/her residence. Provides skills training to promote independence including, but not limited to, maintaining a safe and clean room, laundry skills, personal hygiene, and interpersonal relationships with housemates. Will participate in the necessary skills training for Villagers to meet their goals through the AFLS assessment process. Required Qualifications 1 year of work experience and/or training or equivalent combination of education and experience preferred. High school diploma or GED required. Must be able to pass a literacy test. Must be at least 18 years of age, have a valid Class C Georgia Driver’s License, proof of vehicle insurance, and meet company policy regarding MVR requirements. Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting. Must undergo and pass a drug screening and screening for Tuberculosis. Major Duties and Responsibilities Assist the LPN Nurse Manager as needed to include health maintenance activities. Be knowledgeable on the Medication Error/Progressive Disciplinary process, related to medication errors. Transport or accompany villagers to ER/Urgent Care as needed. Complete bi‑weekly hygiene checklists to ensure proper hygiene with all ADL needs. Maintain and adhere to the laundry and room cleaning schedule for each villager, and provide assistance/training to those who are unable to complete this task on their own. Ensure villagers are appropriately dressed for the weather before leaving the residence. Train and assist Villagers on appropriate social skills, problem solving, etc. Complete and document all care tracking tasks in the Electronic Medical Record. Assist Villagers with ambulating across campus depending on villagers physical abilities/needs. Be knowledgeable of and adhere to villager safety/behavior plans. Provide watchful oversight of Villagers well‑being throughout shift. Immediately report incidents of inappropriate behavior or accidents involving villagers to appropriate staff. Complete weekly inventory of villagers ADL supplies, clothing, and snacks. Communicate and document villager concerns involving behaviors or needs to the villager’s case manager. Assist villagers to create grocery lists with healthy food choices, per specific diets to shop in the community. Perform other duties that may be necessary in the best interest of Annandale at Suwanee, Inc. to meet the personal service and care of the Villagers. Personal Skills and Traits Desired / Physical Requirements Ability to read, write, and speak English. Will have basic knowledge of computer programs (Microsoft Office & Outlook). Ability to problem solve and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. While performing the duties of this job, the employee is regularly required to use hands; use fingers; handle or feel; reach with hands and arms; talk; and hear. The employee frequently is required to sit, stand, and walk; climb or balance; stoop, kneel, crouch, bend, or crawl. Employee is required to lift up to 25 pounds and/or move up to 75 pounds (with assistance). Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. Employee is required to transport (drive) and assist Villagers in and out of vehicles. Employee is required to perform CPR/First Aid. Shifts

1. st – 7:00 am–3:30 pm (weekend only)
2. nd – 3:00 pm–11:30 pm (FT, PT, PRN)
3. rd – 11:00 pm–7:30 am (PRN)
