Description

• Conduct thorough patient evaluations in inpatient or outpatient environments and develop personalized treatment plans.
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function.
• Work hand in hand with nurses, physicians, case managers, and fellow rehabilitation professionals to maximize patient outcomes.
• Guide patients and their families through therapy goals, discharge instructions, and recommended home exercises.
• Maintain detailed documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory standards.
• Participate in interdisciplinary meetings to ensure effective care coordination.

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution.

Certifications

• Active state licensure as a Rehabilitation PT.
