Currently hiring part-time Homecare Private Duty Caregivers for Stockton, CA.

• Part-time (flexible schedule)

• Pay - $20-$25 per hour

 

Caregiver Job Description

 

Companion

• Provide conversation and companionship

• Assist with walking and home physical therapy

• Prepare current and future meals, monitor diet and eating

• Medication reminders

• Assist with bathing, grooming and dressing

• Assist with toileting

 

Home Helper

• Light housekeeping

• Laundry and linen washing

• Accompany to doctors’ appointments

• Running errands for clients

• Assist with paying bills

• Prepare grocery list and do shopping

 

Overnights

• All services of companion position

• Assist with evening routine

• Assist as needed through the night

 

Additional responsibilities

• Assist with other related duties as requested by clients

• Call office with any changes to client’s health, needs, and/or living conditions

• Communication with clients in a respectful manner

• Treat client and client’s property with respect

• Capability to drive or take public transportation to clients home

• Capability to lift a maximum of 25 pounds

 

Qualifications:

• Extensive clean background check

• Fingerprint results

• Current TB test results

• CA Driver’s License / CA ID

• California Home Care Aide Registry

• 3-5 years of experience as Patient Care Assistant

 

Qualifications:

• Graduate of a bachelor's program in social work accredited by the Council on Social Work Education.

  • May have a Master's or doctoral degree in social work.

• Licensed as a licensed clinical social worker by the Board of Behavioral Sciences.

  • Minimum of one (1) year's experience in health care.

• Experience in home health care preferred.

• Current BLS/CPR certification

• Valid CA Driver’s license

Job Description Summary

 

The speech-language pathologist is responsible for implementation of standards of care for speech language pathology services and for adherence to all conditions in the Service Agreement

Job Responsibilities:

1. Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.

2. Identifies functional speech defects and establishes a plan of care to improve the patient's function.

3. Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.

4. Recommends communication devices/aids as indicated.

5. Confers with referring physicians and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.

6. As appropriate, participates in case conferences, in-services and other meetings required to

7. ensure coordinated and comprehensive care.

8. Prepares and submits notes per organization policy.

9. Maintains clinical competency in speech language pathology practice and theory.

10. Identifies patient and family/caregiver needs for other services and refers as appropriate.

11. Instructs patient, family/caregiver and other Organization health care personnel in the

12. patient's treatment regime as indicated.

13. May supervise home health aide in procedures directly related to the speech language

14. therapy plan of care.

15. Performs other duties as required to facilitate the delivery of speech language pathology services
