Job Title: Physical Therapist Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year of Adult Home Health experience License: Must have a valid license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience is required Description: Home health visits to patients' homes. Periodic office meetings are part of the role. Reliable transportation is essential. Current driver's license and agency-required liability insurance are mandatory. Average number of patients per day is 5-6. Contractors will be assigned between 5-10 patients per day. Different types of visits and activities have varying point values. Contractors must achieve an average of 30 points per week. Types of cases include Start of Care and Follow-Ups. Weekend requirements vary by location. Willingness to travel up to 75 miles from the home clinic is necessary. Mileage will be reimbursed at the IRS standard rate. Commute to and from the first and last patient is not billable. Local candidates are accepted. Must not currently be a direct employee of any facility if applying as a traveler. All documentation must be completed within 24 hours of the appointment. Delayed and incomplete documentation may impact follow-up patient care. There are no guaranteed hours with this contract. Weekly productivity expectations are 30 units weekly. Familiarity with the HomeCare HomeBase EMR is required. Candidates must be open to potential on-call rotations.
