Experience: Seven years of experience is required, five of which were in a supervisory role. Must have 2 years of experience in discharge planning and utilization review.

Certifications: CCM Certified, PRI Certification, BLS

Provide Associate Director level supervision to the Care Management Department. Plan and develop programs to improve patient care and employee performance. Monitor performance for patient care and assess units with nurse managers. Communicate and assist with any corrective action. Develop a record-keeping system to record medication errors and counseling. Under general supervision of the Director of Nursing, assist with the planning, organizing, directing, coordinating, and evaluation of nursing service and educational programs within the transitional care division of Care Management. Work together with the transitional care team to reduce 30-day readmissions. Serve the high-risk population of patients post-discharge from inpatient medical/surgical units as well as the Adult Emergency Department. Facilitate the safe return to the community setting. Organize and provide oversight of the care coordination necessary to optimize the individual's health in the community. Perform all duties of the Director of Nursing in his or her absence. Participate in developing policies and standards of nursing service and education in the area of care management and transitional care. Provide oversight and direction to the Accountable Care Managers (RNs) and Care Coordinators (CLWs) as well as clerical and ancillary staff to meet the goals of the transitional care division.
