As a Home Health Physical Therapist, you’ll bring expert care directly to patients, helping them recover, rebuild, and thrive in the environment where they feel most comfortable. What You’ll Do
- Evaluate patients and develop individualized treatment plans
- Provide hands-on therapy to improve mobility, strength, and balance
- Educate patients and families on exercises, safety, and recovery strategies
- Track progress and adjust care plans to maximize outcomes
- Collaborate with physicians, nurses, and care teams
- Empower patients to regain independence and confidence

Requirements
- Active Licensed Physical Therapy license in the state of NM
- Passion for patient-centered, compassionate care
- Strong clinical and communication skills
- Ability to work independently and manage your schedule
- Home health experience is a plus but not required
