Case Manager - Healthcare

Schedule: Days, 8 hours, 40 hours guaranteed weekly

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40

Requirements:

• 1+ year experience in a Medicare-certified home health agency (required)
• Active and clear driver's license and car insurance (required)
• Hawaii license (required)
• Experience with Homecare Home Base and OASIS (required)
• COVID and FLU vaccine (required)
• Bachelor's degree (preferred)
• Case Manager experience (preferred)

Roles & Responsibilities:

• Provide excellent patient care by assisting in the collaboration, development, implementation, revision, and reporting of case management programs.
• Act as a liaison between patients, families, nurses, physicians, and the multidisciplinary team to optimize healthcare outcomes.
• Lead the coordination of patient care with other disciplines, ensuring the appropriateness and timeliness of care.
• Monitor diagnostic testing, treatments, and patient care to ensure alignment with clinical courses and ongoing care needs.
• Collaborate with acute care case managers regarding the appropriateness of ongoing care and treatment plans.
• Manage frequent emergency department visitors by reviewing admission history and coordinating necessary care.

Compliance / Notes:

• Mileage reimbursed at $0.75 per mile.
• On-call requirement: 6 shifts a month, including weekends (2-3 per month).
• Candidates must be able to obtain a Hawaii nursing license in person, as per state regulations.
