Social Worker

Job Description: Under clinical supervision by the Director of Case Management, responsible for providing social work and utilization review services to psychiatric patients and their families. Utilization management includes pre-certification, concurrent and discharge reviews, appeals and denials. Also responsible for assessing the patient's medically related psychosocial, emotional and financial needs and providing assistance to the patient and their family as those needs impinge on the patient's condition, treatment, recovery and safe transition from the hospital to another care environment or to home. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES Identifies new admissions to the psychiatric adult unit and reviews for clinical appropriateness and treatment plan. Develops an individual plan of care based on the psych/social needs of the patient and develops a realistic discharge plan in collaboration with the patient/family/health care team. Implements a plan of care based on the needs of the patient, providing ongoing support and resources to patients and families. Effectively communicates with the interdisciplinary health care team discharge needs and obstacles affecting treatment and discharge disposition. Maintains credentials in clinical social work and attends continuing education programs. Provides support and consultation to other members of the health care team. Other duties as assigned. MINIMUM WORK EXPERIENCE Experience in crisis intervention desired. Clinical Social Worker experience, minimum one year in an inpatient psychiatric hospital setting preferred. REQUIRED LICENSES Current Commonwealth of Massachusetts Licensed Independent Clinical Social Worker (LICSW) license required.Pay Range: $0.00 USD - $0.00 USDThe pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time.
