Responsibilities Provide high-quality nursing care for LTC residents adhering to care plans. Assess resident condition, document changes, and communicate with care team promptly. Administer medications and monitor for adverse reactions per protocols. Ensure compliance with regulatory standards and facility policies at all times. Develop, implement, and update individualized care plans with measurable goals. Collaborate with physicians, therapists, and family members to optimize outcomes. Track metrics and outcomes to meet performance and quality targets. Document and report safety incidents and participate in root-cause analyses. Mentor and educate junior staff to promote professional growth and performance. Assist with admissions, discharges, and transfers ensuring continuity of care. Utilize care pipelines and territory insights to identify opportunities for improvements. Experience/Qualifications Active RN license in the applicable state with LTC experience. Proven ability to develop and update individualized care plans with measurable goals. Experience administering medications and monitoring adverse reactions per protocol. Strong documentation and communication skills for care teams and families. Knowledge of regulatory standards (e.g., state LTC regs, CMS/survey readiness). Competence in incident reporting and participation in root-cause analyses. Proficiency with care pipelines, performance metrics, and data‑driven improvements. Mentor mindset with experience educating junior staff and fostering growth. Ability to assist with admissions, discharges, and transfers ensuring continuity of care.
