CASE MANAGER (full-time)

Case Manager

Jackson Parish Hospital is currently looking for a full-time Case Manager to work in the case management department. This position includes a full benefit package with medical, dental, vision, retirement, accrued time off, holiday pay, life insurance, short and long term disability, along with other supplemental plans.

The Case Manager coordinates and manages services provided to each patient to establish a focused, individualized program geared towards specific goals unique to that patient.

Essential Functions/Job Responsibilities:

Acts as a liaison for patient, family, and staff, overseeing day to day operations of all care provided to ensure successful patient outcomes.

• Enables the patient's program to proceed in an orderly, purposeful, and goal-directed manner through daily contact with the patient and family.
• Attends patient staffing and team meetings, monitoring patient progress, and re-evaluating goals.
• Facilitates the discharge process and arrangements for follow-up and appropriate supportive services.
• Encourages the participation of the patient and family on an ongoing basis in discussion of plans, goals, status, etc. by directing patient and family conferences and through daily communication.
• Works with other staff on insurance reviews and denials if needed.
• Responds appropriately and assists others in emergency situations in a calm and effective manner.
• Maintains and ensures patient confidentiality at all times.
• Communicates with Nurse Manager and DON/CNO regarding patient needs.
• Perform any other task as assigned to by Director of Nursing/Chief Nursing Officer.
• Reports to work on time and as scheduled; completes work within designated time frames.
• Attends staff meetings as scheduled.
• Represents the organization in a positive and professional manner.
• Adheres to JPH policies and procedures.

Requirements

EDUCATION, TRAINING, AND EXPERIENCE:

• Graduate of an accredited school of nursing, BSN preferred; or degree in social work.
• Minimum of 3 years of experience in nursing, case management, or social work preferred.

LICENSURE/CERTIFICATIONS:

• Current RN/LPN license in the state of Louisiana; or currently licensed as a Social Worker in the state of Louisiana.
• Current BLS certification is required upon hire.
• ACLS and PALS certification is preferred.

LANGUAGE and PROFICIENCY REQUIREMENTS:

• Able to communicate effectively in English, both verbally and in writing.
• Strong written and verbal skills.

SKILL REQUIREMENTS:

• Ability to work alone or with minimal supervision.
• Professional/Courteous rapport with patients, families, co-workers, physicians and visitors.
• Ability to work under and handle stress in an appropriate manner.
• Ability to work long hours between breaks and meals, if necessary.
• Basic computer competency. Knowledgeable in Microsoft products including Word and Excel preferred.

MACHINERY/TOOLS/EQUIPMENT REQUIREMENTS:

• Ability to operate and troubleshoot telephone, fax machine, copy machine, computer, and printer/scanner.
